This paper describes a rare case of diabetic myonecrosis in a 63-year-old patient with a long-standing history of type 2 diabetes. The patient presented with progressive pain in the right thigh, difficulty walking, and muscle weakness. Initial examination and imaging suggested a focal myositis, but further investigation including muscle biopsy confirmed the diagnosis of diabetic myonecrosis. The patient improved with glycemic control and pain management. Diabetic myonecrosis is a rare complication of diabetes that can mimic other conditions such as myositis, and prompt diagnosis and management are essential for a favorable short-term prognosis. Este artículo describe un caso raro de mionecrosis diabética en un paciente de 63 años con una larga historia de diabetes tipo 2. El paciente presentó dolor progresivo en el muslo derecho, dificultad para caminar y debilidad muscular. La exploración inicial y las imágenes sugerían una miositis focal, pero investigaciones adicionales, incluida la biopsia muscular, confirmaron el diagnóstico de mionecrosis diabética. El paciente mejoró con el control glucémico y el manejo del dolor. La mionecrosis diabética es una complicación rara de la diabetes que puede imitar otras condiciones como la miositis, y un diagnóstico y manejo oportunos son esenciales para un pronóstico favorable a corto plazo.
