Short-Term Outcomes of Diode Laser Vaporization versus Transurethral Resection of Prostate

Abstract

Abstract Background Laser-based prostate vaporization procedures have become widely accepted as an alternative to transurethral resection of prostate (TURP) for the treatment of benign prostatic hyperplasia (BPH). Objective To compare between short-term outcomes and complications of diode laser vaporization versus bipolar transurethral prostate resection. Patients and Methods Prospective study was conducted on 34 patients with BPH who were enrolled and divided into two equal groups; group A included patients who were subjected to bipolar TURP and group B included patients who were subjected to transurethral laser Vaporization of prostatic adenoma. Results Compared to bipolar TURP, diode LASER vaporization of prostate group showed a larger prostate size (120 gm vs 62gm in bipolar, p < 0.05, respectively), longer operative time (107 min vs 73 min in bipolar, p < 0.05, respectively), higher decrease of postoperayive prostate size (-90 gm vs -25gm in bipolar, p < 0.05, respectively), alower drop of postoperative hemoglobin (Hb) level (-0.5 vs 1 gm/dl in bipolar, p < 0.05, respectively), higher postoperative maximum flow rate (25 ml/sec vs 15ml/sec in bipolar, p < 0.05, respectively), lower rate of early complications (5.9 % vs 23.5% in bipolar, p < 0.05, respectively). There were no major complication in both groups. Conclusion Diode laser vaporization is a safer and more effective method for managing BPH compared to bipolar TURP. It has lower rates of both early and late complications, and results in greater reduction of prostatic size and improved postoperative outcomes. The choice between the two techniques should be based on individual cases.
