Partner impact on the efficacy of continuous positive airway pressure in the treatment of obstructive sleep apnea

Abstract

<bold>Introduction :</bold> Continuous positive airway pressure (CPAP) is the standard treatment for obstructive sleep apnea (OSA). Among the factors influencing adherence to CPAP, the role of the partner has been suggested. The aim of this study was to evaluate the effect of the partner’s involvement with OSA management in CPAP compliance and efficacy. <bold>Methods :</bold> Cross-sectional study was conducted in 2023, including 61 patients followed up at the pneumology department D of Abderrahmane Mami Hospital for severe OSA. Patients were divided to 2 groups: Group1(G1) of involved partners in CPAP compliance and efficacy(n=36) and Group2(G2) of absent or uninvolved partners(n=25). <bold>Results :</bold> The median age was 61 years, with a female predominance at 82%. Forty-six patients were married (10 of whom had an uninvolved partner) and 15 unmarried (2 divorced, 3 single and 10 widowed). Compliance was comparable in both groups (80% vs 96%;p=0.125). Discontinuation of CPAP was similar in both groups (1.6% vs 0.4%; p=0.223). Regular use was higher in G1, with no significant difference (41% vs 30%;p=0.692). The two groups were comparable in terms of average hours of use (5.2 hours vs 5.5 hours ;p=0.601), use over 4 hours (70% vs 68% ;p=0.733) and number of days used (52 days vs 47 days ;p=0.655). Improvement in OSA symptoms was comparable in both groups (97% vs 100%;p=0.999). In G1, Epworth score under CPAP was significantly higher (6 vs 0.5 ;p=0.036). Residual AHI was comparable between the two groups (1.8 vs 1.3;p=0.397). <bold>Conclusion :</bold> Partner’s involvement in OSA management does not significantly influence the compliance, nor the efficacy of CPAP. The role of the partner remains controversial.
