Neuroepithelial cysts are rare, benign, congenital lesions of the central nervous system, accounting for less than 0.4% of intracranial cystic lesions. They may arise in different regions, but midbrain tegmental involvement is extremely uncommon. We present the case of a 50-year-old woman diagnosed with a tegmental neuroepithelial cyst, complicated by triventricular hydrocephalus and motor coordination deficits. This article aims to raise awareness of neuroepithelial cysts, enhance recognition patterns, and reduce the risk of misdiagnosis.
