Aim: This study investigated the use of optical coherence tomography to assess macular structure changes after pars plana vitrectomy (PPV) in patients with diabetic tractional retinal detachment (TRD). Methods: Forty eyes of 40 patients with diabetic TRD who underwent PPV were examined using optical coherence tomography one month after silicone oil removal. The study aimed to identify the types and frequency of macular abnormalities observed on optical coherence tomography. Results: Preserved foveal pits were found in 40% of eyes, while epiretinal membranes (ERMs) were present in 20%. Cystoid macular changes (CCM) and disorganization of retinal inner layers (DRIL) were observed in 50% of eyes each. Ellipsoid zone disruption was detected in 67.5% of patients. Conclusion: optical coherence tomography serves as a valuable tool for detecting various structural changes in the macula after PPV for diabetic TRD. Considering both pre-operative and post-operative OCT findings can potentially improve patient management strategies and predict visual prognosis.
