Lésion Rénale Aiguë au Cours de l’Asphyxie Périnatale au Cameroun

Abstract

ABSTRACT Introduction. Au cours de l'asphyxie périnatale, le rein est l’un des organes le plus fréquemment atteint. L’objectif de cette étude était de déterminer l’incidence de la lésion rénale aiguë au cours de l’asphyxie périnatale et ses facteurs associés. Méthodes. Il s’est agi d’une étude prospective réalisée du 1er janvier au 31 juillet 2021 incluant les nouveau-nés à terme admis pour asphyxie périnatale dans trois hôpitaux de référence de Yaoundé. La lésion rénale était définie comme un taux de créatinine sérique supérieur à 8 mg/l dès le 3ème jour de vie. Les données collectées ont été résumées sous forme de moyenne ± écart-type, fréquences et pourcentages. Résultats. Nous avons inclus 44 nouveau-nés victimes d’une asphyxie périnatale parmi lesquels 19 (43,2 %) avaient une lésion rénale aiguë. Dans le groupe des 19 nouveau-nés avec atteinte rénale, on retrouvait une oligurie (63,2%), une hématurie (26,3%), des œdèmes (10,5 %), une hypotension artérielle (10,5%), une détresse respiratoire (84,2 %) et une hémorragie digestive (31,6 %). Sur le plan neurologique, neuf nouveau-nés (47,4 %) avaient une encéphalopathie hypoxo-ischémique de stade II et huit (42,1%) étaient au stade III. Six nouveau-nés (31,6%) sont décédés alors que treize (68,4%) ont obtenu une récupération de leur fonction rénale. Les facteurs associés à la lésion rénale aiguë étaient : le stade III de sévérité de l’encéphalopathie hypoxo-ischémique et la présence d’une hématurie. Conclusion. En milieu urbain camerounais, la lésion rénale aiguë est fréquente au cours de l’asphyxie périnatale, et est corrélée à la sévérité de l’asphyxie. RÉSUMÉ Introduction: During perinatal asphyxia, the kidney is one of the most affected organs. The objective of this study was to determine the incidence of acute kidney injury during perinatal asphyxia and its associated factors. Materials and Methods: This was a prospective study carried out from January 1st to July 31, 2021 including full-term newborns admitted to three reference hospitals in Yaoundé for perinatal asphyxia. Renal injury was defined as a serum creatinine level > 8 mg/l from the 3rd day of life. The collected data were summarized as mean ± standard deviation, frequencies and percentages. Results: We included 44 newborns suffering from perinatal asphyxia, among whom 19 had acute kidney injury, an incidence of 43.2%. The latter presented with oliguria (63.2%), hematuria (26.3%), edema (10.5%), arterial hypotension (10.5%), respiratory distress (84.2%) and digestive bleeding (31.6%). Neurologically, nine newborns (47.4%) had stage II hypoxic-ischemic encephalopathy and eight (42.1%) were stage III. Six newborns (31.6%) died while thirteen (68.4%) achieved recovery of renal function. Factors associated with acute kidney injury were: stage III severity of hypoxic-ischemic encephalopathy and the presence of hematuria. Conclusion: In urban Cameroon, acute kidney injury is common during perinatal asphyxia, and is correlated with the severity of asphyxia.
