Kartagener syndrome, a subset of primary ciliary dyskinesia, is characterized by the clinical triad of chronic sinusitis, cylindric bronchiectasis, and situs inversus totalis. Gastrointestinal manifestations are rarely described. We report the case of a 34-year-old woman, previously diagnosed with bronchiectasis in childhood, who presented with chronic watery diarrhea. Imaging revealed complete situs inversus, cylindric bronchiectasis, and chronic sinusitis with bony wall thickening, consistent with Kartagener syndrome. This case highlights the importance of recognizing rare gastrointestinal manifestations and the key role of imaging.
