Eosinophilic granulomatosis with polyangiitis (Churg-Strauss syndrome) is a rare multisystem vasculitis in which cardiac involvement represents a major determinant of morbidity and mortality. Myocarditis may result from eosinophilic infiltration, and cardiac MRI is the most sensitive noninvasive tool for identifying these abnormalities. We report the case of a 23-year-old man with a history of asthma and persistent hypereosinophilia who was diagnosed with Churg-Strauss syndrome with renal and neurological involvement. This case underscores the crucial role of cardiac MRI in the evaluation of suspected cardiac manifestations of Churg-Strauss syndrome, enabling timely adjustment of immunosuppressive therapy and improving long-term outcomes.
