High proportion of plasmid-mediated quinolone resistance genes and integrons in hospital- and community-acquired Enterobacterales in Dakar, Senegal

Abstract

This study aimed to determine the distribution of plasmid-mediated quinolone resistance (PMQR) and class 1, and 2 integrons in ciprofloxacin-resistant Enterobacterales isolated at the Aristide le Dantec university teaching hospital in Dakar, Senegal. In the E. coli strains, the most prevalent PMQR gene was aac(6’)-Ib-cr [59/73; 80.8% (95% CI: 70.3, 88.2)], followed by oqx AB [42/73; 57.5% (95% CI: 46.1, 68.2)], qnr B [3/73; 4.1% (95% CI: 1.4, 11.4)], and qnr S [3/73; 4.1% (95% CI: 1.4, 11.4)]. The proportion of the integrase genes was as follows: intI 1 [48/73; 65.8% (95% CI: 54.3, 75.6)], and intI 2 [1/73; 1.4% (95% CI: 0.2, 7.4)]. The most prevalent PMQR gene in K. pneumoniae isolates was aac(6’)-Ib-cr [49/51; 96.1% (95% CI: 86.8, 98.9)], followed by qnr B [34/51; 66.7% (95% CI: 53, 78)], oqx AB [32/51; 62.7% (95% CI: 49, 74.7)], qnr S [9/51; 17.6% (95% CI: 9.6, 30.3)], and qnr D [1/51; 2% (95% CI: 0.3, 10.3)]. All K. pneumoniae strains carried intI 1 [51/51; 100% (95% CI: 93, 100)] whereas [1/51; 2% (95% CI: 0.3, 10.3)] carried intI 2. Six K. pneumoniae isolates [11.8% (95% CI: 5.5, 23.4)] carried a combination [ aac(6’)-Ib-cr + qnr B + qnr S + oqx AB].
