Introduction : La mortalité maternelle liée à l’avortement volontaire constitue un problème de santé publique, en particulier dans les pays en développement. Objectif : Identifier les facteurs associés aux décès maternels liés à l’avortement volontaire. Méthode : Il s'agit d'une étude cas-témoins menée sur une période de sept ans, de janvier 2013 à décembre 2019. Un cas est défini comme un décès maternel lié à l’avortement volontaire, tandis qu’un témoin est une femme hospitalisée pour un avortement volontaire et sortie vivante de l’hôpital. Des régressions logistiques multivariées ont été utilisées pour identifier les facteurs associés à ces décès. L’odds ratio ajusté (ORa) avec son intervalle de confiance à 95 % a été utilisé pour identifier ces facteurs. Le seuil de signification statistique est fixé à p < 0,05. Résultats : Durant cette période, 52 cas et 104 témoins ont été recrutés. L’âge moyen était de 28,38 ± 7,67 ans. La majorité des femmes étaient mariées (87,2 %). La mortalité maternelle liée à l’avortement volontaire a augmenté entre 2013 et 2019 (de 8,9 % en 2013 à 17,0 % en 2019). Les variables suivantes ont été significativement associées aux décès maternels liés à l’avortement volontaire : âge ≤ 18 et ≥ 35 ans (ORa = 2,4 [1,2-5,3] ), l’antécédent de plus d’un avortement (ORa = 7,3 [3,2-34,9]), la connaissance de méthode d’avortement (ORa = 22,9 [2,2-44,5]), l’hypotension artérielle systolique inférieur à 89 mmHg (ORa = 9,4 [1,2-86,9]), la fréquence cardiaque entre 60 à 90 battements par minute (ORa = 9,0 [1,3-78,7]) et les femmes qui ont obtenu une prise en charge chirurgicale (ORa = 5,8 [2,1-9,9]). Conclusion : La planification familiale, la mise en place de services de soins post-avortement et la prise en charge adéquate des patientes peuvent réduire les décès maternels. Introduction: Maternal mortality due to induced abortion is still a significant public health concern, particularly in developing countries. Objective: To identify the factors associated with abortion-related maternal mortality. The following variables were significantly associated with abortion-related maternal mortality: age ≤18 or ≥35 years (aOR=2.4 [1.2-5.3]), history of more than one abortion (aOR=7.3 [3.2-34.9]), knowledge of the abortion method (aOR=22.9 [2.2-44.5]), systolic blood pressure below 89 mmHg (aOR=9.4 [1.2-86.9]), heart rate between 60 and 90 beats per minute (aOR=9.0 [1.3-78.7]), and receiving surgical treatment (aOR=5.8 [2.1-9.9]). Conclusion: The following factors were associated with maternal mortality related to abortion: age ≤18 and ≥35 years, history of more than one abortion, knowledge of abortion methods, systolic blood pressure below 89 mmHg, heart rate between 60 and 90 beats per minute, and surgical treatment. Family planning, post-abortion care services, and appropriate care can reduce abortion-related maternal mortality.
