Abstract Introduction. Neoadjuvant chemotherapy is known to be effective in reducing the stage of locally advanced breast cancer. This study examines the response assessment of breast cancer patients to this therapy in Cameroon. Methodology. A retrospective cohort study was conducted among breast cancer patients attending three cancer care centers in Douala. Clinical, pathological, and therapeutic data were collected from patient records. Results. A total of 118 patients with locally advanced breast cancer were included in the study and evaluated for treatment response to neoadjuvant chemotherapy. The mean age at diagnosis was 47.39±12.3 years, ranging from 22 to 77 years. Half of the patients were 45 years old or younger (50.85%). Invasive ductal carcinoma was the most common histological subtype (76.27%), with Scarff Bloom and Richardson grade II being the most represented (43.90%). T4 tumors were the most prevalent, accounting for 68.65%, and all patients were in a locally advanced stage. The most prescribed chemotherapy regimen comprised 5-fluorouracil, Doxorubicin, and Cyclophosphamide in 43.22% of cases, with 53.39% showing an objective response to treatment. The Luminal B molecular subtype was associated with a poor response to neoadjuvant chemotherapy (p=0.006, OR=8.36, IC=1.81-38.57). Conclusion. Early onset of breast cancer, high histological grade, large tumor size, and a significant proportion of triple-negative tumors are prevalent clinical phenotypes in this population. Identifying genomic variants associated with these phenotypes may lead to new therapeutic approaches for managing breast cancer in Cameroon. Résumé Introduction. La chimiothérapie néoadjuvante est reconnue pour son efficacité à réduire le stade du cancer du sein localement avancé. Cette étude évalue la réponse à cette thérapie chez les patientes atteintes d'un cancer du sein au Cameroun. Méthodologie. Une étude de cohorte rétrospective a été menée auprès de patientes atteintes d'un cancer du sein et fréquentant trois centres de traitement du cancer à Douala. Les données cliniques, pathologiques et thérapeutiques ont été recueillies à partir des dossiers des patientes. Résultats. L'étude a inclus 118 patientes camerounaises atteintes d'un cancer du sein localement avancé, avec un âge moyen au diagnostic de 47,39 ± 12,3 ans. Le carcinome canalaire infiltrant était le sous-type histologique le plus fréquent (76,27 %), principalement de grade II (43,90 %). Les tumeurs T4 prédominaient (68,65 %), confirmant le stade localement avancé chez toutes les patientes. Le régime de chimiothérapie le plus courant (43,22 %) était le 5-fluorouracile, Doxorubicine et Cyclophosphamide, entraînant une réponse objective chez 53,39 % des patientes. Le sous-type moléculaire Luminal B était associé à une mauvaise réponse (p=0,006, OR=8,36). Conclusion. L'apparition précoce du cancer du sein, un grade histologique élevé, une grande taille tumorale et une proportion significative de tumeurs triple-négatives sont des phénotypes cliniques prévalents dans cette population. L'identification des variants génomiques associés à ces phénotypes pourrait ouvrir la voie à de nouvelles approches thérapeutiques pour la prise en charge du cancer du sein au Cameroun.
